We are a team of nurse specialists and doctors who work alongside your GP and other Health Care Professionals involved in your care, to provide a high quality service to assist you in managing symptoms and supporting psychological and emotional needs that may occur as a result of your illness.
The Community Team's background is in palliative care and they also work alongside social workers, occupational therapists and physiotherapists to help the patient cope with the effects that illness may be having on their life.
As part of patient care, the team will undertake an assessment and if needed, deliver ongoing support to the patient or provide support to the GP and district nurse team caring for the patient.
The service offered by the team include:
- Pain and symptom management;
- Emotional Support;
- Co-ordination of services including arranging for Hospice@Home, Hospice admission, volunteer support, DayServices and referrals to external agencies for support;
- Terminal care;
- Bereavement support.
The service is available Monday - Friday, 9am - 5pm and via an on-call system at weekends and bank holidays. Out of hours specialist advice may be obtained from staff on the Inpatient Unit.
As a patient you will be allocated a named nurse, who will assess your needs, plan and coordinate your care.
The team provides services for patients registered with GPs belonging to Westminster PCT.
