Community Support

To gain access to most of the services offered by St John's Hospice, you need to be referred by a medical consultant, GP or specialist palliative care nurse through completion of our referral form.

ImageThe Community Team visit people at home to offer support and advice. The team is multi-disciplinary, consists of clinical nurse specialists with a specialist background in palliative care and doctors, working alongside social workers, occupational therapists and physiotherapists. The aim of the team is to help people to cope with the effects that illness may be having on their life.

We work with GP, district nurses and other community health services to enable people to live comfortably and well at home for as long as possible. As part of a patient's care, the team will undertake an assessment, and, if needed, deliver ongoing support to the patient or provide support to the GP and district nurse team caring for the patient.

The services offered by the team include: 

  • Pain and Symptom Management;
  • Emotional Support;
  • Co-ordination of Services including arranging for Hospice@Home, Respite Care, Hospice Admission; Volunteer Support; Day Service; and referrals to external agencies for support;
  • Terminal Care; and
  • Bereavement support.
The service is available Monday-Friday (0900-1700), and via an on-call system at weekends and bank-holidays. Out of these hours specialist advice may be obtained from staff on the In-Patient unit.

Each patient referred will be allocated a named nurse, who will assess their needs, plan and co-ordinate their care.
